In early April, the UK reported cases of hepatitis of unknown origin being detected in children. Unfortunately, there have also been deaths due to this mysterious disease. Doctors and scientists are still looking for the source of the problem, and the World Health Organization (WHO) urges pediatricians and parents to pay attention to the symptoms of the disease and consult them immediately with specialists. It is also an appeal to Polish parents, because hepatitis of unclear etiology in young patients has already been diagnosed in Poland.

Mysterious hepatitis in children

On April 5, disturbing reports arrived from the United Kingdom. The UK Health Safety Agency said it was investigating cases of strange hepatitis in children. The disease was detected in 60 young patients in England, which greatly concerned doctors and health officials, as so far only a few (seven on average) such cases have been diagnosed each year. Moreover, the cause of inflammation in children was unclear, and infection with the most common hepatitis viruses, ie HAV, HBC and HVC, was excluded. The patients also did not live close to each other and did not move around, so there was no question of an infection center.

Similar cases quickly began to appear in other countries, incl. Ireland, Denmark, the Netherlands, Spain and the USA. Seven weeks after the first information about the mysterious disease, the disease has already been diagnosed in over 600 children in many countries around the world, mainly in Europe (of which more than half in Great Britain).

The course of the disease in most children is severe. Some young patients developed acute hepatitis, and 26 even required a liver transplant. Unfortunately, deaths have also been recorded. So far, 11 victims of the mysterious epidemic have been reported: six of the children were from the United States, three from Indonesia, and two from Mexico and Ireland.

Hepatitis epidemic in children – possible causes

Hepatitis is an inflammation of an organ that occurs under the influence of various factors. In most cases, it is the result of infection with a pathogen, mainly a virus, but inflammation can also be caused by alcohol or drug abuse, improper diet, exposure to toxins, and various diseases, including autoimmune diseases.

In the case of hepatitis currently detected in children, the etiology of the disease is unclear. For obvious reasons, addiction-related factors have been excluded, and the relationship with chronic, hereditary and autoimmune diseases is questionable, as most of the children were in good health before falling ill.

However, proving it will not be easy, because some children could pass COVID-19 asymptomatically, and their bodies may no longer have antibodies.

At the moment, the most likely cause of hepatitis in children is infection with one of the types of adenovirus (type 41). This pathogen has been detected in a large proportion of young patients, but it is not known if it was the infection that caused such widespread inflammation. The uncertainty is compounded by the fact that this adenovirus is not so aggressive as to cause large changes in internal organs. It usually causes the typical symptoms of gastritis, and the infection itself is short-lived and self-limiting. Cases of transition to acute hepatitis are very rare and usually affect children with reduced immunity or after transplantation. No such burden has been found among the currently ill patients.

Recently, an article appeared in The Lancet Gastroenterology & Hepatology, the authors of which suggest that the coronavirus particles may have stimulated the immune system to overreact to adenovirus 41F. As a result of the production of a large amount of inflammatory proteins, hepatitis developed. This may suggest that SARS-CoV-2 led to an abnormal immune response and resulted in liver failure.

Hepatitis in children in Poland – do we have anything to fear?

The first cases of hepatitis of unknown etiology have already been found in Poland. Official data from the National Institute of Hygiene show that 15 such cases have been recently detected, but it has not been specified how many of them concern adults and how many children. However, several-year-olds are among the patients, which is confirmed by the drug. Several children with hepatitis have recently come to my department, most of them are several years old, although there have also been infants. Despite the complete diagnosis, the cause of the disease could not be found. We treated children symptomatically and fortunately we managed to bring them out of the disease. Reluctantly and slowly, but the children recovered

– he informs, adding that the few-year-olds ended up in the ward with various symptoms, incl. persistent fever and dehydration in the course of diarrhea.

When asked about the assessment of the situation related to the increased number of cases of hepatitis in children in Poland, the pediatrician calms down:

- We do not have an emergency situation, but we remain vigilant, because there is certainly something going on that requires such vigilance. So far, we have not had such events that have been recorded in the world that a liver transplant was necessary, and there have been no deaths. These ailments in a child must not be underestimated!

The symptoms of hepatitis in a child are characteristic, but they can be confused with the symptoms of “ordinary” gastroenteritis, the common intestine or gastric flu. Primarily:
